Gilad, my knees and back ache, and I'm worried that starting to exercise will just make it worse. Where do I even begin?

That's a very common concern, and you're smart to be cautious. The key is to start with exercises that build you up without breaking you down.

My number one recommendation is to get in the water if you have access to that. The pool is your best friend! The buoyancy of the water supports your body weight, taking all the pressure off your aching joints while you build strength.

For a full breakdown of this and other great starting points, be sure to read this month's main article, "Move with Ease."

Move with Ease: Your Guide to Exercising with Joint Pain

If you live with joint pain, especially in your knees or back, the idea of exercise can feel like a catch-22. You know that movement is good for you, but the thought of aggravating that familiar ache can be enough to keep you on the couch.

But what if we told you that the right kind of exercise is one of the most powerful tools you have to reduce joint pain?

The key isn't to stop moving; it's to start moving smarter. By choosing activities that support your joints instead of stressing them, you can build strength, improve flexibility, and significantly decrease your daily discomfort. Here’s how to get started.

The Golden Rule: Low-Impact is Your Best Friend

The single most important concept for exercising with joint pain is to go low-impact. This simply means choosing activities that don't involve jarring or pounding motions. Low-impact exercises minimize the force traveling through your knees and spine, allowing you to reap all the cardiovascular and muscular benefits without the pain.

Your Joint-Friendly Exercise Playbook

If you're wondering where to begin, these activities are fantastic for protecting your knees and back while giving you a great workout.

1. Go for a Swim (or a Walk in the Water) The pool is your sanctuary. The buoyancy of water supports your body weight, taking virtually all the pressure off your joints. This allows you to move freely and without pain, making it the perfect environment for anyone with significant arthritis or back pain.

  • Try This: Swimming laps is excellent, but you don't have to be a swimmer to benefit. Simply walking back and forth in the shallow end or joining a water aerobics class provides gentle resistance that strengthens muscles and improves circulation.

2. Get on a Bike (Stationary is Great!) Cycling is a phenomenal way to strengthen the muscles that support your knee joint—namely your quadriceps and hamstrings—without the impact of walking or running. A stationary bike is often the best choice as it provides a controlled, stable environment.

  • Try This: Ensure your bike is set up correctly. Your knee should have a slight bend (about 10-15 degrees) at the bottom of the pedal stroke. Start with no resistance and gradually increase it as you get stronger.

3. Master the Elliptical Machine The elliptical offers the best of both worlds: it mimics the motion of running but your feet never leave the pedals. This creates a smooth, gliding movement that is incredibly gentle on both the knees and the lower back, all while providing an excellent cardiovascular workout.

4. Embrace Strength Training Strong muscles are like shock absorbers for your joints. The more muscle you have around your knees and supporting your core, the less stress your joints have to endure.

  • For Knees: Focus on exercises that strengthen the quadriceps, hamstrings, and glutes. Think straight leg raises, glute bridges, and wall sits.
  • For Backs: A strong core is paramount. A weak core forces your back to do all the work. Gentle exercises like planks (start on your knees if you need to), bird-dog, and pelvic tilts can build stability without straining your spine.

5. Improve Your Flexibility and Balance Stiffness and pain often go hand-in-hand. Gentle stretching, restorative yoga, or Tai Chi can work wonders for improving your range of motion and reducing that feeling of tightness. Plus, these practices enhance your balance, which is crucial for preventing falls that could lead to further injury.

A Quick Word of Caution

You may want to limit or modify high-impact activities like running, jumping, or high-intensity aerobics classes that involve jarring movements, as these can often worsen knee and back pain.

Most importantly, listen to your body. A little muscle soreness after a workout is normal, but sharp, shooting, or sudden pain is a signal to stop immediately.

It’s always wise to chat with your doctor or a physical therapist before starting a new routine. They can help you design a plan that is safe and effective for your specific needs. The first step is often the hardest, but by choosing the right movements, you can transform exercise from a source of fear into your greatest ally in the fight against joint pain.

To your health and mobility,
